Please use this form to apply for a Variation to an Aeroplane or Helicopter AOC. Please also submit the relevant checklists (EU-OPS for aeroplanes and JAR-OPS 3 for helicopters). To apply for a variation to a Balloon AOC please use Form SRG 1311. This Form SRG 1802 replaces Form1240V.
